2nd opinion: Five weeks on the same antibiotic? It is not working and needs to be changed. Maybe antibiotics will not be able to give enough relief for the use of local anesthetic alone and some level of sedation will be necessary to accomplish definitive care. Such sedation is not usually available in most general dentist offices.

Call the DDS: If it is hurting him, follow up with the doctor. No reason to wait until friday.
