Second opinion : Any infection should be resolved by now. Sometimes another dentist may be able to get you numb enough, or can use gas or sedation to help you get the treatment you need.

In my opinion: This has been going on too long. I would seek the opinion of the best oral surgeon in your area. If the tooth if hopeless, it should be extracted as soon as possible. An oral surgeon can give you IV sedation and take the tooth out without you feeling anything.
