More info needed: What else has changed in that time? If you have been on the same dose of Adderall (dextroamphetamine and racemic amphetamine) xr for 3 years, it is unlikely for the medication to suddenly stop working or to cause side effects. However, adding or taking away a different medication or other changes in your health, habits, or circumstances may be a factor. See your doctor to discuss possibilities of what is happening. Labs, other meds, etc.

Stop the Medication: Three years? Are you serious? If the medication is not working any more or the symptoms are intolerable you stop the medication and get back to your doctor to discuss other options.
