Urine collection: Creatinine clearance requieres a full and precise 24 hour urine collection. The results can be falsely low if there is an undercollection, or falsely high if there is an overcollection. Also, creatinine can be affected by a high protein diet (the "ghoulash effect"). Such diets will raise serum creatinine.

Not so: The serum creatinine is not sensitive to detect early disease. A patient may lose ~ 40% of their function before the creatinine goes into the "abnormal" range. Conversely, a 24h urine collection to measure the creatlinine clearance will identify patients with "normal" creatinine, who in fact have early kidney disease.
