Chiari 1 malformatio: If you are not having any signs that the cerebellar tonsils are impacted, then no action is necessary at this time. If you have developed headaches and the course is worsening in frequency or severity, i would see a neurosurgeon.

Chiari surgery: Treatment is based on how symptomatic the chiari malformation is to you. With MRI these are being found in a number of asymptomatic patients. You would need to see a neurologist or neurosurgeon to determine if the chiari is symptomatic and whether it needs treatment via surgery.

Surgery alternatives: If your symptoms are from the chiari, surgery is the best option. To know for sure that it's the chiari causing your symptoms is the hardest part. Sometimes the only way to find out is to proceed with surgery. Neurosurgeons who treat a lot of chiari will develop a good sixth sense for who may get better and who may not (many people have MRI findings of chiari but won't get better with surgery).
