Doubtful: Please see a gastroenterologist. This could be a rectal or anal fissure or something else entirely, get a correct diagnosis and proper treatment. As you know it is a very sensitive area, save yourself unnecessary pain and have it treated professionally. Hope all turns out well for you.

Won't hurt: Sounds like you probably have an anal fissure. Basically a small tear that can be very painful . There are many medical as well as surgical treatment options so best to see your doc. In the meantime, can try stool softeners, site baths.
