DYSPHAGIA: Not always strep infection. Some other conditions like foreign body, viral infection can manifest the feeling of lump in the throat.

Throat lump: It can be associated with acid reflux and i see that you have heartburn and you smoke, which does not help it. The lump may also be from post nasal drip if you have nasal allergies. I will bet on acid reflux, you need to change your diet and stop smoking and maybe take some otc antacids.
