Several options: Tell the doctor exactly what happened, as you did in your question. A fall on the shoulder can cause numerous possible injuries such as a clavicle fracture, proximal humerous fracture, scapula fracture, cervical spine injury, brachial plexus injury, shoulder sprain, etc.
