Pyelo: I am not sure what you're concerned over. If the antibiotics are working, then you need to wait until the infection is killed and hopefully you should start feeling better. If the antibiotics are not working, it usually becomes obvious within 3 days after the start of the antibiotic. If your condition is getting worse on antibiotic, you should go to the er.

See below: Hopefully you had a urine culture done and are on an antibiotic to which the causative bacteria is susceptible. Treatment should last 10-14 days depending on the antibiotic chosen. It is a good idea to have a renal ultrasound done to be sure you do not have any kidney stones or obstruction. Fever and flank pain may take 3-4 days to resolve after start of effective antibiotic rx.
