In those cases we obtain projective psychological testing to determine whether someone has a more subtle psychosis. Many people have psychosis like symptoms but are not actually psychotic. There is also such a thing as a pseudo-psychosis. Please consult with a psychiatrist because this is a very delicate subject. Fortunately the treatments are often quite effective.
