Once there is an opening through the gum to allow saliva (thus bacteria) to reach the surface of the impacted tooth, two things can happen - decay of the tooth or infection of the gum covering it. A cyst or tumor can occasionally develop from an unerupted impacted tooth; it requires a periodic panoramic xray to detect.

Decay: Having an impacted wisdom tooth can lead to pericoronitis or inflammation of the gum tissue around the impacted wisdom tooth. What i also see very frequently is decay of the tooth right in front of the wisdom tooth. This is usually caused by the impacted wisdom tooth lying intimately against the second molar and causing decay as plaque and saliva get trapped between the teeth.
