It differs: Catatonia can result from different causes, e.g., severe psychosis or severe depression. The person's mental state can differ accordingly: stuporous, highly confused, utterly hopeless, psychotic, etc. Although motionless and apparently inert, catatonic states can also be high-anxiety, i.e., too terrified to move. It's impossible to know just by looking.
