Carpal Tunnel: This is all dependent on how you feel. There are multiple treatments for carpal tunnel syndrome, an unpredictable neurologic condition. Night splints, activity modification and injections are all conservative options, though surgical intervention may eventually be needed. An emg/nerve conduction study can help determine the severity of your cts.

If it is bad enough: Carpal tunnel surgery is an option if you have been thoroughly evaluated by a specialist, the diagnosis has been confirmed with nerve studies and you have failed appropriate conservative measures. Having said that, the surgery had a good success rate. If it is becoming very bothersome, it is time to be evaluated.
