Causes vary: On and off pain can be tendinitis which hurts more with movement and grasping. Osteoarthritis is not a consideration at age 20. Rheumatoid arthritis is usually on both sides of the body) associated with swelling, and more constant. Carpal tunnel syndrome is usually felt as numbness, tingling, and/or pain in the hand. A doctor's history and physical exam and limited testing should sort this out.

Variety of issues: Wrist pain can be from a variety of things. Carpal tunnel syndrome typically presents with numbness in the thumb, first and second fingers, and part of the third, and sometimes with wrist pain. One can also have issues with the soft tissue of the wrist, such as tendinitis, or even a hairline fracture of one of the carpal (wrist) bones. See your PCP, or a hand surgeon.
