Usually: They teach us in med school that questions that include, "always" or, "never" are almost always wrong. Most renal cell carcinomas are vascular and will show vascularity. Some may have outgrown their blood supply and undergo areas of necrosis making sonographic demonstration of blood flow difficult. Large body habitus (obesity) can also present a technical challenge.
