Unlikely: An normal thorough echocardiogram, especially a pediatric echo, generally rules out a vsd. It would be very unlikely to fail to identify a significant vsd on a thorough echo. Tiny ones can sometimes be missed, but they usually don't matter anyway since they rarely cause problems or need treatment.
