Low dose: It is likely that your dose needs increased or medication changed / added, or both. If seeing spots, you should call your dr asap for advice on next step--er vs eye dr or urgent office visit.

Not adequate: If your BP is still high then you are not adequately controlled. Your doctor needs to adjust your treatment regimen. Don't wait.

Many reasons : Gosh, i hoped you would list your pills. People with more than mild hypertension usually require more than one pill to control it, sometimes 3 or 4. Some combinations work better than others. Also if you add salt to your food, or take any nonsteroidal arthritis pills like advil or aleve, (naproxen) or are retaining fluid for other reasons, your BP pills wont work as well.
