I've always been underweight, but my diet has included (a lot?) saturated fat (92% dv per day) the past year, should i worry about heart attack?

I will recommend: You to get a lipid panel and check triglycerides, total cholesterol, HDL and LDL cholesterol. Also, balance your diet more with more proteins, complex carbohydrates, vegetables and less unsaturated fats. Being skinny do not always translate into being healthy. I will avoid saturated fats at all.

See doctor: You need to see your doctor he can check your lipid profile if you have chest pain angina and symptom of CAD he will do stress test withecho/ myo view and so on ? See your doctor if you worry.
