Mostly observation: If the nail plate was avulsed cleanly and the nail bed (matrix) has not been damaged, then only observation is necessary. Keep the area clean and dry. The nail plate usually takes about 6 months to grow from the cuticle to the tip of the digit. If you have any questions or concerns, be sure to see a board certified hand surgeon.
