Depends: It really depends on how the nail was avulsed and whether the entire nail was avulsed. If this was a crush injury and there is a laceration to the nailbed, then that should be surgically repaired. Also, if not treated properly, the eponychial fold (proximal nail fold) can scar to the sterile matrix preventing nail re-growth. These are typically simple injuries best treated by a hand surgeion.

Most nail injuries: such as an avulsion look worse than they are. the nail plate, the hard nail comes off but if the underlying nail bed is ok, the nail grows back in a few months. laceration to the sterile nail matrix or the germinal matrix (the root of the nail can affect growth

Mostly observation: If the nail plate was avulsed cleanly and the nail bed (matrix) has not been damaged, then only observation is necessary. Keep the area clean and dry. The nail plate usually takes about 6 months to grow from the cuticle to the tip of the digit. If you have any questions or concerns, be sure to see a board certified hand surgeon.

Wound Care: Wash the area twice a day with soap and water. Keep the area dry and covered for the rest of the day. The area is going to be very sensitive for a couple of months.
