Serious discussions: This is a matter for serious discussions between you, your doctors, and your family. In a few states this type of thing may be legal, but still extremely controversial. Psychiatric consultation would be part of the process. You may want to focus instead on creating a living will, and discussing your wishes with the person who would serve as your health care proxy, should you be incapacitated.
