Some of the important factors are age at first dislocation, associated injuries, number of dislocations, knee malalignment, and generalized "loose ligaments", etc. A doctor needs to assess these factors prior to determining which treatment is appropriate.

Yes: Many factors come into play. Usually conservative treatment with bracing and pt is affective for acute, non-recurrent patellar dislocation. Recurrent dislocations and some acute dislocations may require surgical repair of the medial patellofemoral ligament or re-alignment.
