Benign: And malignant causes. If you are straining/constipated-then: hemorrhoids. There are other causes. See your doctor to help figure out cause.

Rectal bleed: If you have hemorrhoids, you know it, these are cause for small bleed. Or there is constipation & straining can do that. However if hemorrhoid & constipation are not there and you see blood on each evacuation, then see gastroenterologist for colonoscopy to find out the other causes such as diverticulosis , avm, ischemia, colitis of any type&cancer.
