Arrythmia.: The missing pulse followed by a strong one occurs with an abnormal, premature beat. This type of arrhythmia is common in otherwise healthy people, and is ok as long as it's rare and there are no other symptoms. Smoking (and caffeine) may be cause. If associated with chest pain, shortness of breath, lightheadedness or dizziness it could be dangerous. If frequent, your doctor should do an ecg.
