Possible causes: 1. Tight shoes ( look at length and width) 2. Capsulitis of the 2nd toe joint , may also cause nerve compression, inflammation 3. Lower back issue, tarsal tunnel syndrome (these are not likely) 4. See # 1. (very common issue). Look for athletic shoes that come in various widths. Recommend a specialty running shoe store where someone can measure your feet properly.

Back or swelling: It may be from your back as lower back radiculopathy can translate to symptoms in the foot (l5/s1 areas). Also, it could be a ligament tear that causes swelling to the area. The localized swelling and trauma may impinge on the nerves around that area. Most likely from your history, it's probably from your back.

Numb: As the other dr's mentioned radiculopathy etc etc are all possibilities. One other is compression neuropathy . It is possible because you state your foot goes numb with athletic shoes, that something in the shoe fit or position of the shoe is compressing a blood vessel(s) serving the nerve(s) to the foot. A thorough evaluation by your podiatrist is in order
