Musculoskeletal?: Pain in occipital region with upright posture suggests effect of a 10 lb head stressing fascia, muscles, & tendons that support head against force of gravity. Chronic muscle spasm and soft tissue inflammation are of concern. Irregular head/neck posture might be explanatory. Osteopaths and physiatrists have skill to diagnose. A recent lumbar puncture can also arouse such symptoms.

Muscle tensioin: Many patients clench or grind their teeth at night for various reasons- stress, sugar, drugs and bad bites. The result is 56 pair of muscles in the head and neck become like a charley horse or muscle spasm and result in referred pain. See a dentist that understands temporomandibular disease and get a diagnosis.
