Anal fissure: A anal fissure, a tear in the mucous lining of the anus, usually results from excessive stretching, as you have with severe constipation. The treatment is to relieve the pressure. Take a laxative that softens the stool (miralax, metamucil, lactulose) and you can use medicines that relax the muscle spasm underneath the tear (which is what causes the pain), eg. Topical Nitroglycerin (prescription).

Hurts, but treatable: Anal fissure is a crack or tear in the anal canal, exposing the sphincter muscle, which spasms, which causes the pain when trying to move bowels. Treatment focuses on spasm relief to allow healing. Topical nitroglycerine or Diltiazem is used frequently. Botox can be used if fissure persists. Surgery can affect continence, but may be needed if other treatments fail to cure.
