Anxiety disorder: You have had a very thorough cardiac work-up including echo, ekg... You may have an anxiety disorder that is causing the symptoms you are describing. Are you under stress or pressure? Probably time to look for other explanations than cardiac - your heart is fine.

Very doubtful: It is extremely doubtful that you have a heart problem with all of that testing coming out normal. To be honest, it is much more likely to be related to stress/anxiety in your life, or possibly due to less than optimal lifestyle behaviors (diet, caffeine, exercise frequency, etc).
