Unlikely: Either that bug has to be extremely small or your perforation very large. Not likely.

Rare: It's reasonably rare to get a bug in your ear in the first place. *theoretically* a bug could get through a perforation and if it did you'd need to see an ENT as many of the treatments to kill the bugs (numbing medicine, alcohol etc) are not safe in the setting of a hole in the ear drum, making the retrieval more difficult. I would not lie awake thinking about this rare likelihood event.
