See below: Several factors may reduce skin elasticity and contribute to sagging including genetics, weight gain/loss (especially rapid), sun damage and smoking. If the change in form was sudden it may be due to implant rupture.

Lift with implants: After a lift with implants the surgery will restart the aging process. Depending how strong your scar tissue is will determine how long your lift lasts. Large breasts (d cup) will eventually sag. I would return to your surgeon and see what they think can be done.

Maybe...: Whether or not the “significant sagging” is to be expected depends on what has happened to your body over the course of the past 3 years. For example, pregnancy and/or weight gain/loss can significantly change breasts' position on a woman's chest wall. Sometimes, genetics and skin elasticity may play important roles. No but not rare : Following a breast lift. It takes about 6-9 months to achieve a natural stable result. Thereafter sagging may depend on several factors including weight gain/loss, pregnancy, genetics and amount of natural breast tissue. However the size ( a 600cc implant is more likely to sag than a 250cc) of the implant and the position, particularly over the muscle, may make this more prone to sagging.

It can happen...: ...But this is not typical. Your chances are increased by larger breast volume and poor skin tone. Women with small breasts rarely need a breast lift, while women who receive breast reductions always do. By using the smallest breast implants that will get the results desired, the results are prolonged. If the natural breast tissue is falling off the implant, then the issue is likely skin tone.
