What to do if i get sleepy all the time and always have headaches. what's wrong?

Sleepiness: If you are sleepy all the time with headaches also, it will be good to get a brain MRI rule out any neurologic cause of this. Also a in lab sleep study will rule out sleep apnea or other disorders which can also cause sleepiness and headaches.
