Disturbing: These symptoms are too non-specific to allow an opinion for cause, but they are sufficiently severe to warrant a full detailed assessment by a pcp. It is not possible to know whether this has an organic basis or a psychological one, but a good clinician should be able to work this out. Good luck and hope you get better soon.
