Genetics and environ: Diabetes is due to destruction of islet cells in the pancreas which make Insulin and Insulin resistance in the tissues. It is part due to genetics especially for type 1 diabetics and environment with high fat/carb diet, lack of exercise and genetics in type 2 diabetics.
