However, with some medical conditions, genetic factors, or not being at the right level of "blood thinness" (inr), another clot can occasionally form.

Yes: Although anticoagulants are supposed to treat blood clots by allowing the body to resolve them, and are supposed to prevent further clot formation from occuring; this can occur albeit rarely.
