Sometimes: Blood thinners prevent future blood clots from forming and help the body dissolve the current blood clot in conditions such as deep venous thrombosis (leg clot), pulmonary embolism (lung clot), and cardiac thrombus (heart clot). However, with some medical conditions, genetic factors, or not being at the right level of "blood thinness" (inr), another clot can occasionally form.

Yes: Although anticoagulants are supposed to treat blood clots by allowing the body to resolve them, and are supposed to prevent further clot formation from occuring; this can occur albeit rarely.
