Lots of reasons: Pulsatile tinnitus can be caused by lots of things, such as: wax impaction ear infection fluid in middle ear or mastoid a mass in the middle ear space such as a "glomus" tumor aneurysm hydrocephalus (increased pressure in head) increased blood flow through the vessels from conditions such as hyperthyroidism or anemia if it troubles you, get it checked out.
