Tongue infection: Three problems I have seen in my practice. First is patient is unable to remove rings, causing trauma to the tissues. Second is infection from non-sterile placement or proper cleaning. Third and most concerning is damage to teeth requiring invasive and expensive restoration of teeth, that would not have been needed if not for piercings.

Possibly: I have seen a number of cases where these body adornments have chipped teeth requiring filling and in one case a crown. So be careful.

Lots of problems: Tongue barbells can chip or fracture front teeth. Gums can be damaged by the rubbing of barbells requiring periodontal surgery. If the barbell becomes unscrewed it could be aspirated into the lungs. Infections can be quite severe & eventually lead to disfigurement. There can be significant blood loss & permanent neuralgia (pain). Safe? - the risk of complications is too significant.

Are they bad?: Yes. Pierced area can become infected/scarred/unsightly requiring antibiotic therapy/plastic surgery. Sometimes fixture and tissues seem to knit together, requiring surgical removal. Tongue fixtures are infamous for causing fractured teeth and/or gum destruction. The greater the damage, the more extensive/expensive the repairs.
