Many things: A history & physical will help greatly in the true diagnosis. It could be a simple cyst, or a "hematoma/contusion" if he hit his head, or an infected pimple/abscess/hair follicle, etc. He should see a dr. And get the proper diagnosis, as the treatment will depend on it. Good luck
