Narcolepsy: Check with your physician for the possibility that you may have developed other conditions like hypothyroidism, obstructive sleep apnea, depression, organic brain disorders, lack of exercise or poor sleep habits. He may need to reevaluate your medications. Hope you feel better soon.

Narcolepsy & Provigi: This is a weak stimulant and ta small degree of tolerance would not be surprising after 7 years of use. Some experts use doses up to 400 mg which may or may not add anymore benefit. Regardless, side effect profile not significantly changed from 200 to 400 mg. Addition of other meds or food hindering its metabolism is another possibility to consider. Co-morbid depression in another consideration.
