TMJ problem?: Sounds like this could be related to a problem with the temporomandibular joint. If all other diagnostic options have failed, a dynamic TMJ MRI exam could potentially be useful in getting to the cause of your problem. An ENT specialist at an academic center would be the best to see regarding this.
