Gradually : Depends on cause: needle stick, catheterization, knife, gunshot wounds, motor vehicle accident, vascular surgery can all have delayed pseudoaneurysms. Other name is pulsatille hematoma that starts with injury and persists in the surrounding tissue and expands over time.
