No pain during opera: You should have no pain during surgery. There will likely be some pain after surgery that is usually easily controlled with medications.

Not very: There is some initial discomfort form the local anesthetic injections. After the operation there will be some soreness that can easily be controlled by the medications your surgeon prescribes.

Local anesthesia: Either an anesthesiologist or your surgeon can administer a local anesthetic block (penile block) to reduce the pain of the procedure and to last for several hours after. The block itself, however, can be quite painful. Another alternative is a spinal or epidural anesthetic which will make the surgery completely painless and you remain wide awake (if you choose!)
