Not 100%: No vaccine by itself is 100%, but the tetanus vaccine is one of the most effective shots we give. If you had one 3 years ago, you should be well protected for tetanus. If you are freaking out though, it does not hurt you in any way to get a booster at 3 years to be maximally sure, but it is probably overkill. Keep the wound clean and monitor closely.
