Only if: It's much too tight. Why are you on estrogen and spironolactone. If you're overweight and have Insulin resistance that can easily lead to hair loss. Talk it over with your doctor.

Usually: Usually there's hair loss at areas of tension on the hair which is usually the frontal hair line when wearing a ponytail.
