Toenail: Could be a fungus or from an old injury to the nail matrix that makes it fall off. Check with podiatrist.

Yes: Yes, get evaluated fully and get a definitive diagnosis. You could have a pathology that needs immediate treatment or at least have it trimmed down so it doesn't fall off as much. Hope this helps.

Sounds like fungus: Keep in mind... The fda has recently come out with a blanket statement that toenail fungus can not be cured. It is "managed" by a number of modalities, but it can always come back in the future. There are also many other possibilities as well, so have it examined in person by a podiatrist.
