NSAIDS: Ibuprofen or naproxen. Other agents like diclofenac come in creams you can rub into your knees. They have side effects, read about them.

See details: This is really a terrible situation because effective options are so limited. What caused you to have such severe knee issues at such a young age? If the knees are truly bone-on-bone, nsaids are not likely to help although they are worth a try. So are injectable lubricants such as synvisc (hylan g-f 20) and several others. From an orthopedic standpoint, osteotomies may be worth a consideration.
