Delusional disorder: I am a little confused by part of the question. So i will attempt to provide clarification on one aspect of it. There are many people with delusional disorders who in general function well. Others might not be aware that they have a delusion. On the other hand, someone who is really focused on & acting on their delusions might suffer significant impairment.

Delusional disorder: Delusions are false beliefs that are firmly held despite evidence to the contrary, and not typical of the person's culture, faith, or family. They can be part of formal thought disorders (schizophrenia) but can also appear in people who function well except in the delusional area. In the latter case, the delusions are plausible rather than bizarre -- but are still false thoughts. Meds may help.
