Unregulated supply: Deaths from heroin almost always relate to the uncertainty of supply. Heroin is commonly diluted (termed cut) with bitter substances with no effect but the amount of heroin varies. Rarely some gets in the supply which is high dose which causes respiratory standstill and death. Of course it is an unregulated, illegal market with gangsters killing each other and sometimes the users.
