See a physician: A dark circle on the skin overlying the spine, which developed 2 - 3 years ago and changes appearance, really should be evaluated by your primary care doctor and possibly a dermatologist. It may turn out to be benign, but better to be safe. Chances are the spot on the skin is not related to the back pain, which will require separate evaluation and is usually benign at the age of 21.
