if a woman pregnancy exam went positive in the first try, is she pregnant? and is it necessary to take a blood test exam in order to confirm it ?

Nope : Trust the test. Positive = pregnancy.

Blood test not : Needed. A positive urine pregnancy test is generally reliable. If you have any other signs/symptoms of pregnancy, there is no need for blood test.
