How do they know murmur is innocent if pulmonary valve is not well seen on echo, would doppler show signs even if valve was not seen well?

Typically yes.: Frequently in adults, the pulmonic valve is difficult to see in 2d mode, but spectral and color flow doppler characteristics provide information on the diagnosis and quantification of any pulmonic insufficiency and pulmonic stenosis.
